Find a practitioner
The Licensee Profile and Status feature on this website is a tool you can use to check on the training, credentials, and disciplinary status of practitioners licensed by the State Medical Board of Ohio.
To look up a licensee, follow these steps:
On the Licensee Profile and Status page, in the first box next to "-or- Name (Last, First)," enter the licensee's last name. Tab to the next box and enter the licensee's first name or initial. You may use either capital or lower case letters. If you are unsure about proper spelling, or want to broaden your search, enter the last name only, or try entering just the first few letters of the last name. Click on "Search."
Choose and click on a highlighted name from the list of matches that will appear at the bottom of the page. (NOTE: If you can't find the name you are looking for, or no results are found, try the search again using a different spelling or entering only the first few letters of the last name.) When you click on a highlighted name, the Licensee Profile page will appear. This page includes biographical and professional information about the licensee, as well as details of any public disciplinary action that has been initiated and/or taken.
The lookup system can display a maximum of 100 possible matches in response to asearch request. You can narrow your search, making it easier to find the profile you seek, by completing additional search fields to specify the licensee's city, specialty, etc.
To look up a Physician Assistant Supervision Agreement:
On the Ohio E-License Center page, select "Medical Board" from the drop down box next to the word "Division."
Then select “41. PA Supervision Agreement” from the drop down box next to the word “Professions/Institution.”
In the box next to "BusinessName/DBA, " enter the last name and then the first name or initial, separated by a comma, of the doctor whose plan you are looking up (e.g., "Smith, Joan" or "Smith, J"). You may use either capital or lower case letters. If you are unsure about proper spelling, or want to broaden your search, enter the last name only, or try entering just the first few letters of the last name. Click on "Search."
In the alternative, if you know the Supervision Agreement number (e.g., 41.9999), you may enter that in the box next to” –or-License Number” and the plan profile will be displayed.
If you are checking a particular Physician Assistant, the list of plans that individual is listed under is available by accessing that Physician Assistant’s record. To find this information about a licensee, follow these steps:
On the Ohio E-License Center page, select "Medical Board" from the drop down box next to the word "Division."
Then select “50 Physician Assistant” from the drop down box next to the word “Professions/Institution.”
In the first box next to "-or- Name (Last, First)," enter the last name of the individual you are looking up. Tab over to the next box and enter the individual's first name or initial. You may use either capital or lower case letters. If you are unsure about proper spelling, or want to broaden your search, enter the last name only, or try entering just the first few letters of the last name. Click on "Search."
Choose and click on a highlighted name from the list of matches that will appear at the bottom of the page. This page includes biographical and professional information about the licensee, including the physicians who are approved to supervise the Physician Assistant.
