Lost wallet card

Before a new wallet card can be issued, you must complete an affidavit form. Your signature on this affidavit must be notarized and returned to the Board. Upon receipt of your affidavit, processing will be initiated for the issuance of your new card. You must submit the original affidavit form directly to the Ohio Medical Board at 30 E. Broad Street, 3rd Floor, Columbus, Ohio 43215-6127. WE DO NOT ACCEPT COPIES OR FAXES. THERE IS NO FEE FOR A DUPLICATE WALLET CARD.

It will be necessary that you include your reason in the appropriate section on the affidavit form.

Lost wall certificate

To obtain a duplicate certificate you must complete an affidavit form before a Notary Public and return it to this office with a money order or check made payable to the Ohio Treasurer, Richard Cordray for $35.00. THIS FEE IS NOT REFUNDABLE.

The Board wishes to stress the importance of MAKING A THOROUGH SEARCH before we issue you a duplicate certificate. The Medical Board members are concerned with the problem of physicians misplacing or losing their wall certificates and consider it a serious problem. If there is any possibility that your certificate may have been stolen, please contact the police.

Processing of a duplicate wall certificate takes approximately seven (7) to ten (10) days. You must submit the original affidavit form directly to the Ohio Medical Board at 30 E. Broad Street, 3rd Floor, Columbus, Ohio 43215-6127. WE DO NOT ACCEPT FAXES OR COPIES. If you have any other questions, please contact Debby at Debby.Jones@med.state.oh.us
