Charitable Event Participation Form

This form is to be used for notifying the board of your intent to practice as a volunteer, without remuneration, during a charitable event lasting not more than seven days. This form must be submitted no less than seven calendar days before the first day of the charitable event.

If it is less than seven days from the event start date, you are not permitted to participate in the event and should not complete the form.
